Service Agreement / Ordering Products:

  • If you are a NDIS participant please add items to cart and checkout with Payment Method, "Bank Deposit".
  • In the order notes, please indicate if you are Self-Managed, Plan-Managed or Agency-Managed, your first and last name, NDIS number, date of birth, support budget and support item reference number.  Please note support categories we are registered for above.
