Olympic 142802 is the No. 624 tax invoice and statement book: a carbon duplicate with 100 leaves in the 200 x 125mm size.

Each form is headed Tax Invoice / Statement with a note to delete as appropriate, so the same book covers a bill for work done and a statement of an account. Hamelin Brands, who own Olympic, publish the form artwork: a date line, To and From, ABN lines for both recipient and supplier, an order number box, then a Qty, Description, Price, G.S.T. and Total table finishing with a total inclusive of G.S.T. and a GST amount box.

The blue cover is printed With Extra Carbon. Slip the carbon behind the form you are writing on and the copy underneath stays in the book for your records while the original goes to the customer.
