Olympic 140863 is the No. 638 order book, a carbon duplicate with 100 leaves of purchase order forms at 200 x 125mm in a blue cover.

Each form is set out for ordering from a supplier. Under the Order heading and the form number come date, To and From lines, then the printed instruction please supply the following items, quote the above number on invoice. A Qty and Description table takes the order itself. The foot of the form has an I hereby quote ABN line with room for the authorised person to print their name and sign.
